Subject Access Request Form
Please complete this form if you wish to make a subject access request for personal data we hold about a data subject. The data subject is the individual whose personal data you are requesting. If you are the data subject you will need to complete this section and the data requested section. If you are not the data subject you will also be required to complete the "requester details" section.
Do complete the relevant sections below. Your request will normally be processed within one calendar month upon receipt of a fully completed form and proof of identity.
Please be aware that, unless you indicate otherwise, we always intend to provide our response by email to the email address you have indicated. If you would like to request our response to be provided by post then do detail this in the "data requested" section of the form.

Use this form if you are seeking to make a freedom of information request to the school. Please note this is not the same as making a subject access request for personal data. Please use the subject access request form for a request for personal data.
Please complete the relevant sections below. Once a valid request has been received, the school have 20 school days (or 60 working days if this is shorter) to comply with the request. We will inform you of the deadline on receipt of the request.
